Our Admissions Process

Thank you for considering Park Avenue Extended Care Facility for your rehabilitative and post subacute care. Park Avenue is committed to bringing our residents the highest standards of care through progressive clinical care. This is how we are one of the most trusted facilities on Long Island, and it is what sets us apart from others in the field.

 

To accomplish this goal, we employ highly trained doctors who are leaders in their fields, maintain cutting-edge medical and therapy equipment, and offer a wealth of enriching programs to help facilitate the healing process.

 

Admission to a nursing home can be an adjustment. Here’s a checklist to help you through the process.

 

Documents needed:

​

  • Proof of medical insurance

  • Social Security card

  • Medicare or Medicaid card

  • State Driver’s license or photo ID

  • Copies of Advance Directives

  • Power of Attorney documentation

  • Copy of vaccination card if applicable

 

Suggested clothing and personal items for admission:

​

  • Comfortable clothing for 5-7 days

  • If participating in therapy, a pair of non-skid supportive shoes

  • We provide standard toiletries for our guests; you are welcome to bring your favorite brands from home.

 

**All personal belongings should be inventoried with front desk personnel upon admission**

Please note that laundry services are provided free of charge

 

We understand that choosing a health care provider can be stressful and intimidating. The move-in part of admission can be made easier by touring our facility. At Park Avenue, the warmth of our team is apparent immediately after you step through the door.  Visits from family and friends are always welcomed and encouraged. Plenty of rest is key to a healthy recovery so please be mindful of those resting during nighttime hours.

 

Upon admission, members of our team will meet with you to discuss recovery goals and other pertinent information. A patient’s experience goes far beyond the healthcare that they receive. It encompasses the whole mind and body and addresses their social and emotional health as well. We would like to thank you again for entrusting us with your care and will strive to provide you with the highest quality service available.
