A Smooth Transition to Healing

Admission to a nursing home can be an adjustment. The move-in part of admission can be made easier by touring our facility. At Park Avenue, the warmth of our team is apparent immediately after you step through the door. Visits from family and friends are always welcomed and encouraged. Plenty of rest is key to a healthy recovery so please be mindful of those resting during nighttime hours.

Upon admission, members of our team will meet with you to discuss recovery goals and other pertinent information. A patient’s experience goes far beyond the healthcare that they receive. It encompasses the whole mind and body and addresses their social and emotional health as well. We would like to thank you again for entrusting us with your care and will strive to provide you with the highest quality service available.
